Patient-Centered Medical Home (PCMH)
The Patient-Centered Medical Home is a care delivery model in which patients’ treatment is coordinated through their primary care physicians to ensure they receive the necessary care when and where they need it, in a manner they can understand.
The objective is to have a centralized setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patients’ families. (American College of Physicians)
Primary Care Collaborative
The Primary Care Collaborative (PCC) is a non-profit membership organization that works to advance an effective and efficient health system centered on primary care and the patient-centered medical home. Members include physician and nursing groups, patient and consumer organizations, health plans, employers, industry, quality organizations, and health systems. PCC’s mission is to advance “comprehensive primary care to improve health and health care for patients and their families by convening and uniting stakeholders around research, care delivery and payment models, and policies.” A current initiative is PCC’s Better Health – Now Campaign that “seeks to increase investment in primary care and ensure that primary care payment is aligned with quality outcomes to meet the needs of everyone in all communities.”
American College of Physicians
The American College of Physicians (ACP) is the largest international medical specialty organization focused on internal medicine. Its vision is “to be recognized globally as the leader in promoting quality patient care, advocacy, education and career fulfillment in internal medicine and its subspecialties.” A key area of advocacy is health equity and ACP is committed to be an organization that is anti-racist, diverse, equitable, and inclusive.
