Hospital Compare Centers for Medicare & Medicaid Services (CMS) recently updated its Hospital Compare website to provide information on hospital return days (number of days a patient spends back in a hospital after getting discharged). A new video provides information on hospital return days, and why this is important for patients and families to know about! PCPCC SAN ‘Primary Care Innovations’ Conference Scholarships The Patient-Centered Primary Care Collaborative (PCPCC) is offering registration scholarships to advisors or staff from TCPI practices to attend its Annual Conference, Primary Care Innovations: Putting Shared Principles into Practice, being held in Washington, DC on October 11-12, 2017.
